Treating our clients from the Inside Out

There is a difference between the traditional ‘Outside In’ massage which utilises a mechanical leverage response versus the method we promote at The Massage Centre in Chiswick called ‘The Inside Out’ approach that stimulates nerve impulses to the brain to achieve muscle melt. Both achieve muscle relaxation. But one method’s benefits lasts longer and re-trains muscle behaviour to relax when not in use.

The difference between the “Outside In” and “Inside Out” approaches to massage lies in their techniques and their primary mechanisms of action:

  1. Traditional Outside In Approach (Mechanical Leverage Response):
    • Mechanical pressure and manipulation of hands/forearms applied to tense muscles force tissue to stretch in the opposite direction, triggering increased physiological space that breaks up adhesions and improves blood circulation to promote relaxation, pain relief, and improved range of motion.
    • Requires a massage therapist’s strength to force and coax lost movement patterns in stuck tissue.
    • Benefits feel relieving but will expire over time as the tissues revert to the muscle memory state.
  2. Inside Out Approach (Stimulating Nerve Impulses):
    • This approach targets the attention of the brain using nervous system signals by stimulating sensory receptors in the skin and muscles with
    • appropriate touch to induce therapeutic effects throughout the body.
    • By activating the nervous system, this approach can induce a “muscle melt” sensation, where muscles relax deeply, and tension is released.
    • The goal is to repeat this neurological response over a course of massage treatments to teach or myelinate neurons in the brain so that they can adopt a default state of muscle relaxation. This default relaxed muscle state increasingly replaces the old dysfunctional muscle tension state of muscles that are not in use.
    • Requires a massage therapist to apply their relaxed body weight onto their client’s tissue and allow gravity and the neurological muscle melt response to activate the physiological release of tension as the therapist steers the movement generated by the disappearing tension.
    • Benefits last longer when the mind/body connection establishes new neural pathways over a course of treatment.

Both approaches can use lighter or deeper touch massage techniques. The inside out method normally stimulates clients to exhale sighing breaths as tension releases. This good pain feeling is what we call the therapeutic edge sensation. Generally, ‘Inside Out’ therapists encourage more client breathing and articulate the joints of a body when they are treating it to reinforce neural pathways to the brain. The more connection restored between the mind and body the more our bodies can regulate function, recovery and energy spend.

In summary, the “outside in” approach primarily involves direct manipulation of soft tissues to produce physical changes, while the “inside out” approach focuses on stimulating nerve impulses to induce relaxation and therapeutic effects throughout the body. Both approaches can be effective in promoting health and well-being, and the choice between them often depends on the therapist’s experience and ability to stimulate muscle responses. Clients who know or sense the difference between these approaches often describe ‘inside out’ therapists as “having the touch”, so to speak.
